Arrived at the No. 2 CIBD (Canadian Infantry Base Depot) in Etaples, France and Taken On Strength of the 21st Battalion

After leaving the base depot, he joined the CCRC (Canadian Corps Reinforcement Camp) in Villers au Bois, France

After leaving the reinforcement camp, he joined the 2nd CDRB (Canadian Division Reinforcement Battalion)

After leaving the reinforcement battalion, Private Donnelly joined the 21st Battalion in billets in Ourton, France

 

Apr 4, 1918

While in the front line south-east of Agny, France, Private Donnelly received a shrapnel wound to his right arm and he was evacuated to the No. 6 CFA (Canadian Field Ambulance) for first aid before being transported to the No. 3 CCS (Casualty Clearing Station).  As a result of having a tourniquet on his arm for 8 hours, his arm had to be amputated above the elbow

Transferred to the No. 24 Canadian General Hospital in Etaples, France

 

Apr 7, 1918

Invalided to England aboard the Hospital Ship Stad Antwerpen

 

On arrival in England, he was admitted to the 2nd Western General Hospital, Lily Lane, Moston, Manchester

Transferred to the EORD (Eastern Ontario Regimental Depot) for pay purposes while in hospital

Transferred to the East Lancashire Red Cross Hospital at Didsbury College, Manchester

 

Apr 30, 1918

Transferred to the Granville Canadian Special Hospital in Buxton

Discharged from the CEF in Kingston, Ontario

He was discharge to the care of the SCR (Soldier’s Civil Reestablishment) Department for continuing medical care

Patrick Donnelly died while a patient of the Ottawa Civic Hospital and was buried in the Notre Dame Cemetery in Ottawa
