Medical exam for the MSA (Military Service Act) of 1917 held at Kingston, Ontario

 

Feb 21, 1918

The 3rd Regimental Draft embarked the SS Melita in Halifax, Nova Scotia

 

 

Mar 4, 1918

Disembarked in Liverpool, England and proceeded to Seaford where the draft was absorbed into the 6th Reserve Battalion to continue training

 

Aug 9, 1918

Transferred to the 21st Battalion

Arrived at the CIBD (Canadian Infantry Base Depot) in Etaples, France and Taken On Strength of the 21st Battalion

 

Aug 15, 1918

After leaving the base depot, Private Fyke joined the 21st Battalion in Fouquescourt, France

 

Oct 11, 1918

During the capture of the town of Iwuy, France, Private Fyke received a bullet wound to his right leg that shattered his femur bone.  Overnight he was evacuated to the No. 9 CFA (Canadian Field Ambulance)

 

Oct 12, 1918

Transported to the No. 30 CCS (Casualty Clearing Station) where his leg had to be amputated, leaving him with a 3 ½ inch stump.

 

Oct 14, 1918

Transferred to the No. 11 Stationary Hospital in Rouen, France

 

Dec 9, 1918

Invalided to England aboard the Hospital Ship St. Andrew

 

On arrival in England, he was admitted to the No. 4 Canadian General Hospital in Basingstoke

Transferred to the EORD (Eastern Ontario Regimental Depot) for pay purposes while in hospital

 

May 9, 1919

Transferred to the No. 5 Canadian General Hospital in Kirkdale, Liverpool

 

Jun 10, 1919

Embarked the Hospital Ship Essequibo in Liverpool

 

 

Jun 21, 1919

Disembarked in Portland, Maine, USA and proceeded to Kingston, Ontario where he was Taken On Strength Military District No. 3 Casualty Company

 

Jun 22, 1919

Admitted to the Queen’s University Military Hospital in Kingston

 

Jun 23, 1919

Granted leave until July 7, 1919

 

Jun 26, 1919

Posted to the Queen’s Military Hospital and remained as a patient

 

Jul 31, 1919

Transferred to the No. 2 District Depot in Toronto, Ontario and admitted to the DOH (Dominion Orthopedic Hospital), Christie St., Toronto.  While here, he was supplied with a peg leg and an artificial leg for him to use.

 

Dec 29, 1919

Reported to be AWL (Absent Without Leave) from the hospital

 

Jan 6, 1920

Reported back to the hospital at 10 pm and was admonished for his absence and forfeited 9 day’s pay

 

Jan 15, 1920

Discharged from hospital and from the CEF in Toronto, Ontario

Ø  War Service Badge Class “A” issued number 221358

Ø  Proposed residence on discharge Chesterville, Ontario

Following his discharge, the British War Medal and Victory Medals were sent to him at Chesterville, Ontario

Religion given at that time was Episcopalian

William Fyke served as a rural mail carrier for 25 years after his discharge

 

Mar 17, 1950

William John Fyke died at home in Chesterville, Ontario and was buried in the Maple Ridge Cemetery, Maple Ridge, Ontario
