After leaving the base depot, he joined the CC Rein C (Canadian Corps Reinforcement Camp)

Shortly after leaving the reinforcement camp, Private Hart joined the 21st Battalion at the front

 

Aug 28, 1918

During the advance on the Sensee River, south of Vis en Artois, France, Private Hart received a superficial shrapnel wound to his hip and was evacuated to the No. 5 CFA (Canadian Field Ambulance) for first aid before being transported to the No. 42 CCS (Casualty Clearing Station)

 

Aug 29, 1918

Transferred via the No. 5 AT (Ambulance Train) and admitted to the No. 56 General Hospital in Etaples, France

 

Sep 26, 1918

Invalided to England aboard the Hospital Ship Stad Antwerpen

 

On arrival in England, he was admitted to the Fort Pitt Military Hospital in Chatham and Arthritis was added to the diagnosis

Transferred to the EORD (Eastern Ontario Regimental Depot) for pay purposes while in hospital

 

Sep 30, 1918

Transferred to the No. 11 Canadian General Hospital in Shorncliffe

 

Oct 17, 1918

Transferred to the Canadian Convalescent Hospital in Woodcote Park, Epsom

 

Oct 23, 1918

Discharged from hospital and granted sick leave until November 4, 1918

Attached to the 1st CCD (Canadian Command Depot) in Witley for physical training and exercise on completion of his leave
