While moving into Brigade Reserve in Fouquescourt, France, Private Hayes received a shrapnel wound to his shoulder and was evacuated to the No. 11 CFA (Canadian Field Ambulance) for first aid before being transported to the No. 48 CCS (Casualty Clearing Station)

Transferred via the No. 15 AT (Ambulance Train) and admitted to the No. 1 Australian General Hospital in Rouen, France

 

Aug 28, 1918

Transferred via the No. 8 Ambulance Train and admitted to the No. 74 General Hospital in Trouville, France

 

Sep 28, 1918

Transferred to the No. 13 Convalescent Depot in Trouville

 

Oct 15, 1918

After being discharged from the convalescent depot, he reported to the Canadian Infantry Base Depot in Etaples, France

 

Oct 23, 1918

After leaving the base depot, he joined the Canadian Corps Reinforcement Camp in Aubin St. Vaast, France

 

Oct 28, 1918

After leaving the reinforcement camp, Private Hayes rejoined the 21st Battalion in Auberchicourt, France
