Admitted to the No. 5 CFA (Canadian Field Ambulance) with a diagnosis that reads Influenza.  He was transferred the same day to the to the North Midlands Casualty Clearing Station at Mont des Cats and placed in their Divisional Rest Station

Discharged to duty with the 21st Battalion from the rest station

Admitted to the West Cliff Canadian Eye and Ear Hospital in Folkestone with a diagnosis that reads Myopia and Gastric Instability.  Glasses were ordered to correct his vision of 6/60 in both eyes.  Bismuth and soda were prescribed to stop him vomiting and settle his stomach.

Leave extended to June 13, 1916 by Medical Certificate

Transferred to the Canadian Training Depot and posted to the General List of Officers for pay purposes while in hospital

SOS (Struck Off Strength) the 21st Battalion following admission to hospital
