After leaving the base depot, he joined the CC Rein C (Canadian Corps Reinforcement Camp) in Calonne Ricouart, France

After leaving the reinforcement camp, Private Johnson joined the 21st Battalion in the front line near Acheville, France

Rejoined the 21st Battalion in the Cellars Camp at Neuville St. Vaast, France from the field company

Admitted to the 8th Australian Field Ambulance with a diagnosis that reads PUO (Pyrexia of Unknown Origin), a fever without a known cause.  He was transported the same day to the No. 53 CCS (Casualty Clearing Station)

Transferred via the No. 23 AT (Ambulance Train) and admitted to the No. 16 General Hospital in Le Treport, France

 

Sep 5, 1918

Invalided to England aboard the Hospital Ship St. Denis

 

On arrival in England, he was admitted to the Lord Derby War Hospital in Warrington

Transferred to the EORD (Eastern Ontario Regimental Depot) for pay purposes while in hospital

 

Nov 11, 1918

Transferred to the Military Convalescent Hospital in Woodcote Park, Epsom and Gonorrhea was added to the diagnosis

 

Nov 19, 1918

Transferred to the Canadian Special Hospital in Witley and the diagnosis reads Gonorrhea and VDS (Venereal Disease Syphilis)

Discharged from hospital and transferred to the 6th Reserve Battalion in Seaford
