Discharged to duty from hospital

Left Camp Borden by train for Halifax Nova Scotia

 

Aug 21, 1916

Embarked the RMS Olympic at Halifax Nova Scotia

 

Aug 30, 1916

Disembarked at Liverpool England and proceeded to the Witley Camp

 

Oct 15, 1916

Transferred to the 116th Battalion at Bramshott

Posted to the 21st Battalion

Arrived at the CBD (Canadian Base Depot) at Le Havre France and TOS the 21st Battalion

Discharged from hospital and posted to “A” Company at the CBD for those recovering from wounds and illness

Joined the 2nd Entrenching Battalion at Hersin

Left the 2nd Entrenching Battalion to join the 21st Battalion

Joined the 21st Battalion in the front line trenches near Thelus, with the Battalion Headquarters in Maison Blanche

During the advance on Vimy Ridge, Pte Perkins was Killed in action

Following the war the British War Medal and Victory Medals were sent to his step mother, Sarah Perkins, at 12 Orchard St, London Ontario 

The Plaque (Dead Man’s Penny) and Scroll were sent to his father, Charles Perkins, at the same address 

There was no Memorial Cross issued, as his mother had pre-deceased him
