Attested into the 21st Battalion in Kingston, Ontario

Admitted to the Queen St. Military Hospital diagnosed with Influenza.

Discharged to duty from hospital.

Embarked the RMS Metagama in Montreal, Quebec

 

May 15, 1915

Disembarked in Devonport, England and proceeded to the West Sandling Camp, near Hythe, Kent.

Embarked the St. Seiriol in Folkestone

 

Sep 15, 1915

Disembarked in Boulogne, France and proceeded to St. Omer.

 

Feb 9, 1916

Granted 9 days leave.

 

Feb 20, 1916

Rejoined the battalion from leave.

Admitted to the No. 8 CFA (Canadian Field Ambulance) after his right hand was accidentally scalded.

Transferred to the No. 9 CFA.

Discharged to duty from the CFA.

 

Apr 9, 1917

During the advance on Vimy Ridge, Private Thomas received shrapnel wounds to his right leg and was removed to a field ambulance for treatment.

 

Apr 15, 1917

Invalided to England aboard the Hospital Ship Princess Elizabeth.

Posted to the EORD (Eastern Ontario Regimental Depot) while in hospital.

 

Apr 16, 1917

Admitted to the Military Hospital in Dover and noted as being seriously ill.  The decision was made to operate and amputate the lower 1/3 of his right leg.

 

May 2, 1917

Transferred to the Fort Pitt Central Military Hospital, Chatham.

 

May 15, 1917

Transferred to the VAD (Volunteer Aid Detachment) Shipstead Hospital, Sevenoaks, Kent.

 

Jul 5, 1917

Transferred to the Fort Pitt Central Military Hospital, Chatham.

 

Jul 6, 1917

Transferred to the Granville Special Hospital in Ramsgate.

 

Aug 15, 1917

Transferred to the Canadian Military Hospital in Liverpool.

 

Aug 16, 1917

Due to complications and lack of healing, additional surgery was performed to do a further amputation.  This was done to approximately 6 inches above the right knee.

 

Sep 10, 1917

A note was added to his file to also notify Percy C. Thomas, brother, 621 Princess St., Kingston, Ontario of his progress.

 

Sep 14, 1917

Invalided to Canada aboard the Hospital Ship Araguaya, embarking in Liverpool.

 

Sep 25, 1917

Disembarked in Quebec City, Quebec and proceeded to Military District No. 2, Toronto, Ontario to be fitted with an artificial limb and was admitted to the Davisville Military Hospital.

 

May 30, 1918

Granted leave with subsistence until June 13, 1918.  His address while on leave is recorded as 275 Laurier Ave., W., Ottawa, Ontario.  That address continued to be used for each time he proceeded on leave as shown below.

Granted leave with subsistence until August 17, 1918.

Leave with subsistence extended until September 21, 1918.

Died in the Veteran’s Pavilion of the Ottawa Civic Hospital, Ottawa, Ontario and was buried in the Pinecrest Cemetery, Ottawa.
